Overseen by a non profit board that includes both community leaders and patients, each of our locations is a federally qualified community health center that strives to serve people from all backgrounds.Services offered at this location are: Family Medicine, Immunizations, Family planning services, Acute Care, Integrated Behavioral Health, Traditional Behavioral Health, Laboratory, Pharmacy Assistance, Preventative Family Dental Care (Cleanings, X Rays, Exams), Family Dentistry (Fillings, Extractions, Dentures, Crowns), COVID Vaccinations. Transportation is available to get patients to and from their appointments at Great Lakes Bay Health Centers.
